Our assessment take place face to face or via teleconference. All appointments take place in the most appropriate environment for your needs, this could be your home or workplace.
The Occupational Therapist will bring the assessment documents and any relevant tools to complete the assessment. It is helpful if you can have your current medical documents (Certificate of Capacity, imaging reports), medication list and / or current NDIS plan available. This information is relevant to your assessment and will assist in ensuring the correct information is being recorded.
The time taken depends on your needs and goals, service or the type of assessment you require. Don’t forget, that we are here to help! So, if you need to have a break within an assessment, a leg stretch whilst driving, a coffee after your groceries, or an ice cream stop after the basketball we are more than happy to assist.
Please contact us as soon as possible if you require rescheduling or have to cancel your appointment with us. We ask that you give us, at a minimum, 48 hours written notice prior to your appointment for any adjustments/cancellations. Pending the details of your circumstances, you may incur partial or full cancellation fee if written contact has been made before the 48 hours notice window before the scheduled start time of our agreed appointment. Supporting documentation will be required to
negotiate or dispute any cancellation fees incurred.
Payment can occur via our website or via an invoice. Invoices will be sent to either your Support Coordinator, Case Management Specialist or yourself. If you have access to NDIS funds, you can utilise your ‘Improved Daily Living’ funding, under Capacity Building. Payment is required prior to receiving your NDIS Occupational Therapy reports and prior to booking adhoc, one-off appointments.
